INTRODUCTION
With the unprecedented rise in the use of injectable fillers, an increasing number of patients are approaching their physicians to undergo soft tissue augmentation. It is absolutely essential to educate patients about the procedure, its potential benefits, and its potential complications. As always, this should be done prior to the procedure to be most effective. Detailed pre- and postprocedure instructions are an essential part of this education process. It is this education that ultimately empowers patients and helps them to achieve their ideal cosmetic outcome.
Effectively written pre- and postprocedure instructions serve multiple important functions in educating patients regarding their cosmetic procedures. First and foremost, the instructions help patients to understand what to expect from the procedure. Second, they educate patients regarding potential adverse events and possible complications from procedures. Third, they educate patients regarding potential medications and activities that may compromise the postprocedure period or even the cosmetic outcome of the procedure. Finally, they allow the physician to instruct the patient in the management of simple and common postprocedure issues and events that may occur.
In order for these instructions to be of the most benefit, it is essential that they be written in layman's terms. Instructions with complex medical terminology will only serve to confuse the patient, increase their anxiety regarding the procedure, and decrease their likelihood of achieving their desired outcome. The instructions should always be reviewed prior to the procedure with the patient.

Extracellular recordings were made from the isolated goldfish retina during superfusion with various serotonergic agonists and antagonists to determine the effects of these drugs on the maintained activity and response properties of the ganglion cells. Superfusion of the retina with serotonin (25–500 μM) increased the maintained activity of OFF-center ganglion cells and decreased the maintained activity of ON-center ganglion cells. In addition, serotonin also attenuated the excitatory responses to annular stimuli, suggesting a decrease in the strength of surround input to the ganglion cells. The effects of serotonin on OFF-center ganglion cells were mimicked by the nonselective 5-HT1, agonist 5-MeOT and the 5-HT1 receptor agonist 8-OH-DPAT, while only 5-MeOT mimicked the action of serotonin on ON-center ganglion cells. The effects of exogenously applied serotonin on the ganglion cells could be blocked by the mixed 5-HT1/5-HT2 receptor antagonist methysergide but not by the 5-HT2 receptor antagonist mianserin or the dopamine receptor antagonist haloperidol.
These results support previous anatomical and biochemical evidence that serotonin functions in a neurotransmitter or neuromodulatory role in the teleost retina and suggest that serotonin may be involved in modulating the maintained activity and surround input to the ganglion cells. The results also indicate that two different types of receptors may mediate the actions of serotonin in the ON and OFF pathways, respectively.

Introduction
The cosmetic uses of botulinum toxin (BoNT) are the most commonly used of its applications. Interest started after the effect of BoNT was shown in the treatment of blepharospasm and the first description of botulinum toxin for treatment of glabellar frown lines was in 1992. At that time, the use of this potent neurotoxin for cosmetic indications was an interesting footnote to treatments for strabismus, torticollis and other dystonias. Subsequently, physicians began to study and use the botulinum toxins for a variety of cosmetic indications. Today, BoNT is the most commonly performed cosmetic procedure in the world. Understanding how these toxins are used in this arena is essential to any discussion of the botulinum toxins.
Dilution of the toxin for cosmetic purposes
For the purposes of this chapter, the dilution of BoNT will be described in units of the BOTOX® brand of type A toxin. Oculoplastic specialists usually inject using a 1 ml dilution per 100 units of BOTOX®, whereas dermatologists and plastic surgeons vary in their practice towards a general range from 1 ml to 4 ml per 100 units. Variations in concentration affect the concentration gradient between the toxin and its environment. In the forehead, for instance, a dilute concentration may be preferable in order to increase migration, but, in general, clinicians use lower volumes to minimize the risk of this getting into unplanned areas.
